Big Sun Adult Soccer League, through its affiliation with Florida State Soccer Association (FSSA) and the United States Soccer Association (USASA), maintains limited coverage against soccer related injuries for all players who are duly and correctly registered with the league.  This insurance is intended to be a supplemental to primary health insurance as the limits of coverage are quite low.

In the unfortunate event of an injury at a Big Sun Adult Soccer Leauge game, practice or scrimmage, the injured player should follow the following procedures:

1)  It is the injured players sole responsibilty to obtain and complete the insurance form in accordance to the forms directions and the FSSA insurance claims procedures.

2)  Use this insurance form and complete it entirely:  USAA Insurance Forms

3)  Submit the insurance form pursuant to the FSSA insurance claims procedures:  FSSA Insurance Procedures

4) The injured player will deal directly with the insurance company.  The insurance company’s claim examiner, upon determination of a viable claim, will notify the injured player where to send all applicable bills and to whom all player and physician inquiries are to be directed.  After submitting the claim form, the injured player can follow up with:
